The submitter’s name, postal code, address, and telephone number Returning-side/Front: The submitter’s postal code, address, and name Returning-side/Back: Blank * Invitees will be determined by lot if the application number exceeds the limit. * Transfer and resale of the invitation is prohibited. * The postage price will increase starting on Oct 1.
